How you will make an impact:
As a Referral Development Coordinator, your primary responsibilities include Crest Services to the community, managing referrals, updating client profiles, collaborating with interdisciplinary teams and ensuring smooth transitions for new members. The coordinator will work closely with case managers, assistant managers, and interdisciplinary teams, fostering a collaborative work environment. During the admission process, the Referral Development Coordinator plays a crucial role in ensuring that clients receive the necessary services and support.
Schedule Varies: Monday - Friday 8:00 am - 4:30 pm
What you will need:
- Must be 21 years or older.
- Valid driver's license and insurance.
- Ability to maintain confidentiality and detailed records.
- Experience with Therap and knowledge of HCBS admission process
