What You'll Do

Coordinate and oversee the interdisciplinary MDS assessment and care planning process
Ensure timely and accurate completion and transmission of MDS assessments
Maintain compliance with federal, state, and Medicare regulations
Collaborate with nursing and interdisciplinary teams to support quality outcomes
Monitor assessment timelines and address changes in resident condition
Support documentation accuracy and reimbursement integrity

What You'll Need

Required:

Active, unencumbered RN license (Wisconsin, or an active out-of-state RN license you can transfer)
At least 1 year of recent MDS nursing experience
Strong clinical judgment and attention to detail
Able to prioritize and manage multiple deadlines
Effective communication and teamwork skills
