What You’ll Do:
As a MDS Coordinator (RN), you’ll play a vital role in providing high-quality care to our residents. You’ll:
- Coordinate and oversee the full interdisciplinary assessment and care planning process of the Minimum Data Set (MDS).
- Ensure accuracy and timeliness of all documentation submitted in the MDS, in compliance with state and federal regulations.
- Collaborate with clinical and interdisciplinary team members to gather and review resident assessments.
- Submit MDS data according to established facility policies, procedures, and regulatory timelines.
- Monitor and verify that care plans reflect each resident’s current needs and support optimal outcomes.
- Promote each resident’s ability to attain or maintain their highest practical level of physical, mental, and psychosocial well-being.
- Uphold the mission and core values of American Baptist Homes of the Midwest (ABHM) in all aspects of care and communication.
What You’ll Bring:
- Registered nurse (RN) with current, unencumbered, active licensure with the State Board of Nursing in state(s) in which he/she practices
- Minimum 2 years skilled nursing experience required
- Certified ICD-10 coding Preferred
